How it's thought to work

Adropin exerts its influence through a variety of complex mechanisms. In the context of fibrosis, adropin operates by inhibiting TGFβ-induced fibroblast activation. This involves deactivating GLI1-dependent profibrotic transcriptional networks, a process confirmed in both *in vitro* and *in vivo* models, and linked to its receptor GPR19 (PMID 38536934).

For glucose metabolism, adropin is thought to alleviate insulin resistance by reducing hepatic glucose production (PMID 35955453). It also appears to improve glucose utilization by sensitizing insulin signaling pathways, such as Akt phosphorylation, and activating the glucose transporter 4 receptor in mice (PMID 35955453).

In cardiovascular health, adropin is believed to improve endothelial function, modulate lipid profiles, and reduce oxidative stress (PMID 39045137). It achieves a protective effect on vascular endothelium by increasing the expression of endothelial nitric oxide synthase (PMID 35955453). Its ability to reduce serum triglycerides, total cholesterol, and low-density lipoprotein cholesterol while increasing high-density lipoprotein cholesterol also contributes to its cardiovascular benefits (PMID 35955453).

Interestingly, adropin expression also widely reflects the activation of oxidative metabolic pathways and suppression of ribosomal functions and cell division (PMID 40578684). The ENHO gene, encoding adropin, contains retinoic acid receptor-related orphan receptor response elements (RORE), suggesting its expression is under circadian control, impacting the 'liver clock' and showing strong local relationships with circadian genes in most non-hepatic tissues (PMID 40578684).

The catch: what we still don't know

While the findings on adropin are undoubtedly exciting, the research is still in its early stages. Many of the promising results, particularly regarding its mechanisms of action and therapeutic potential, are derived from *in vitro* (cell culture) and *in vivo* (animal) studies (PMID 39888316, PMID 38536934, PMID 35955453). The exact functions and full clinical significance of adropin in humans are not yet completely specified and require further study (PMID 31756066). For example, while lower levels of adropin are associated with conditions like obesity and hypertension (PMID 31598832, PMID 31756066), the direct therapeutic applications and optimal strategies for modulating adropin levels in human patients are still under investigation. The interplay between adropin and other metabolic markers, such as GLP2, also needs more clarification (PMID 31598832). This means that while the horizon looks promising, a great deal more research is needed to translate these fascinating discoveries into clinical practice.
